Din aceast\u0103 categorie fac parte: <i>pemfigoidul bulos<\/i>, <i>herpesul gestationis<\/i> \u0438 <i>pemfigoidul cicatricial<\/i>.<\/p>\n<p style=\"text-align: justify;\"><em>Pemfigoidul bulos<\/em> este o boal\u0103 cronic\u0103 pruriginoas\u0103 debilitant\u0103, ce apare\u00a0\u00een principal la persoanele \u00een v\u00e2rst\u0103 \u015fi este caracterizat\u0103 prin dezvoltarea unor vezicule pe o zon\u0103 eritematoas\u0103 sau urticarian\u0103. Erup\u0163ia este de obicei generalizat\u0103. Au fost descrise mai multe asocieri ale pemfigoidului bulos cu alte afec\u0163iuni, dar probabil c\u0103 acestea sunt \u00eent\u00e2mpl\u0103toare. De asemenea nu a putut fi demonstrat\u0103 asocierea cu procese maligne<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Anticorpii IgG anti-membran\u0103 bazal\u0103 se g\u0103sesc \u00een serul a 80% dintre pacien\u0163i, dar \u015fi local la nivelul membranei bazale, unde se pot observa depozite lineare de IgG \u015fi C3<sup>1;2;3<\/sup>.<\/p>\n<p style=\"text-align: justify;\">\u00cen pemfigoid, anticorpii IgG se leag\u0103 de proteine specifice ce intr\u0103 \u00een alc\u0103tuirea unor complexe de adeziune celul\u0103-matrice-hemidesmozomi\u00a0\u015fi activeaz\u0103 complementul. \u00cen urma reac\u0163iei dintre anticorpi \u015fi moleculele din componenta jonc\u0163iunii celulare sunt eliberate numeroase substan\u0163e chemotactice care atrag la acest nivel celulele inflamatorii; celulele elibereaz\u0103 proteaze \u015fi diver\u015fi mediatori, care degradeaz\u0103 hemidesmozomii \u015fi determin\u0103 o separare dermo-epidermic\u0103. Tinta autoanticorpilor serici sunt proteinele transmembranare BP230 \u015fi BP180 (BPAG1 \u015fi BPAG2), denumite astfel deoarece greutatea molecular\u0103 a acestor antigene este 230 kD \u015fi respectiv, 180 kD. Se consider\u0103 c\u0103 BP180 reprezint\u0103 \u0163inta direct\u0103 a autoanticorpilor datorit\u0103 localiz\u0103rii sale de-a lungul membranei bazale, \u00een timp ce anticorpii anti-BP230 sunt produ\u015fi secundar<sup>1;2<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><em>Pemfigoidul cicatricial<\/em> afecteaz\u0103 \u00een principal cavitatea bucal\u0103 (gingivita descuamativ\u0103) \u015fi zona ocular\u0103 (conjunctivita la debut care dezvolt\u0103 \u00een timp aderen\u0163e fibroase conjunctivale, obliterarea sacului conjunctival, xeroftalmie, opacifierea \u015fi perfora\u0163ia corneei \u015fi \u00een final, orbire). Pacien\u0163ii prezint\u0103 acela\u015fi tip de anticorpi IgG circulan\u0163i ca \u015fi cei cu pemfigus generalizat<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\">De\u015fi diferitele boli cutanate buloase au anumite caracteristici specifice care le disting, de cele mai multe ori manifest\u0103rile clinice sunt atipice \u015fi se suprapun cu alte afec\u0163iuni. Depistarea anticorpilor anti-membran\u0103 bazal\u0103 \u00een ser este extrem de sensibil\u0103 \u015fi specific\u0103 pentru pemfigoid \u015fi reprezint\u0103 un element important \u00een diagnosticarea acestei afec\u0163iuni.<\/p>\n<p style=\"text-align: justify;\">Diagnosticul \u00a0prompt \u015fi precis este util \u00een stabilirea tratamentului, minimizarea sechelelor bolii, comorbidit\u0103\u0163ii \u015fi disconfortului pacientului<sup>2<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><b><i>Recomand\u0103ri pentru determinarea anticorpilor anti-membran\u0103 bazal\u0103 epidermic\u0103<\/i><\/b><b> <\/b>-<b> <\/b>diagnosticul pacien\u0163ilor cu pemfigoid<sup>2<\/sup>.<b><\/b><\/p>\n<p style=\"text-align: justify;\"><b><i>Preg\u0103tire pacient <\/i><\/b><i>-<b> <\/b><\/i>nu este necesar\u0103 o preg\u0103tire special\u0103<sup>3<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><b><i>Specimen recoltat <\/i><\/b><i>- <\/i>\u0432\u0435\u043d\u043e\u0437\u043d\u0430\u044f \u043a\u0440\u043e\u0432\u044c<sup>3<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><b><i>Recipient de recoltare <\/i><\/b><i>- <\/i>vacutainer f\u0103r\u0103 anticoagulant, cu\/f\u0103r\u0103 gel separator<sup>3<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><b><i>Prelucrare necesar\u0103 dup\u0103 recoltare<\/i><\/b> \u2013 se separ\u0103 serul prin centrifugare; se lucreaz\u0103 serul proasp\u0103t; dac\u0103 acest lucru nu este posibil, serul se p\u0103streaz\u0103 la 2-8\u00b0C, la -20\u00b0C sau la -70\u00b0C<sup>3<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><b><i>Volum prob\u0103<\/i><\/b> \u2013 minim 2 mL ser<sup>3<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><b><i>Cauze de respingere<\/i><\/b> <b><i>a probei <\/i><\/b>-<b> <\/b>specimen hemolizat, ser lipemic sau contaminat bacterian<sup>3<\/sup>.<b><\/b><\/p>\n<p style=\"text-align: justify;\"><b><i>Stabilitate prob\u0103 <\/i><\/b><i>-<b> <\/b><\/i>serul separat este stabil <i>7 zile<\/i> 2-8\u00b0C; <i>timp \u00eendelungat<\/i> la -20\u00b0C; nu decongela\u0163i\/recongela\u0163i<sup>3<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><b><i>Metod\u0103 <\/i><\/b>- <span style=\"text-decoration: underline;\">imunofluorescen\u0163\u0103 indirect\u0103;<\/span> ca substrat\u00a0 sunt folosite sec\u0163iuni de esofag de maimu\u0163\u0103; dilu\u0163ia ini\u0163ial\u0103 a serului este de 1\/10<sup>3<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><b><i>Valori de referin\u0163\u0103<\/i><\/b> \u2013\u00a0anticorpi antimembran\u0103 bazal\u0103 epidermic\u0103 \u2013 negativ<sup>3<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><b><i>Raportarea \u015f<\/i><\/b><b><i>i i<\/i><\/b><b><i>nterpretarea rezultatelor<\/i><\/b><\/p>\n<div style=\"text-align: justify;\">Rezultatele sunt exprimate ca negativ\/pozitiv\/echivoc<sup>3<\/sup>.<\/div>\n<div style=\"text-align: justify;\">O parte semnificativ\u0103 a pacien\u0163ilor cu pemfigoid bulos prezint\u0103 \u00een ser anticorpi IgG anti-membran\u0103 bazal\u0103, \u00een timp ce \u00een pemfigoidul cicatricial anticorpii sunt \u00eent\u00e2lni\u0163i \u00eentr-un num\u0103r mai mic de cazuri<sup>2<\/sup>.<\/div>\n<p style=\"text-align: justify;\"><b><i>Limite \u015fi interferen\u0163e<\/i><\/b><\/p>\n<div style=\"text-align: justify;\">Un rezultat negativ nu exclude prezen\u0163a pemfigoidului<sup>3<\/sup>.<\/div>\n<div style=\"text-align: justify;\">Anticorpii serici anti-BMZ pot fi, de asemenea, detecta\u0163i la pacien\u0163ii cu epidermolisis acquisita bullosa (EBA), precum \u015fi la cei cu lupus eritematos bulos.<\/div>\n<div style=\"text-align: justify;\">Deoarece un titru detectabil de anticorpi poate fi g\u0103sit foarte rar \u015fi la persoanele normale, interpretarea rezultatelor trebuie f\u0103cut\u0103 \u00een corela\u0163ie cu manifest\u0103rile clinice<sup>1;2;3<\/sup>.<\/div>\n<p style=\"text-align: left;\" align=\"center\"><strong><span style=\"font-size: 10pt;\">\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/span><\/strong><\/p>\n<div><span style=\"font-size: 10pt;\">\u00a01.\u00a0Thomas P.
